You would like the other caregiver of a child to be able to make an appointment for a visit with a doctor through mojePZU.
- Go to moje.pzu.pl. If you do not have an account in the portal – please register. If you have an account – enter your email address and password. Click Log into.
- In the top right corner of the screen, click your name, the from the drop-down list, select My profile.
- In the MY CHILD section, next to the child’s data, click Add data of the second caregiver.
- Fill out the form with data of the second caregiver who has an account in the moje.pzu.pl service (the data must exactly match those which the second user provided while registering the account) and click Send.
