Authenticate your account on mojePZU

Confirm your identity to have full access to your medical documentation.
View referrals to see a doctor or to have tests, view doctors’ recommendations and collect e-prescriptions without leaving home. 
 
We care for the safety of our patients, so in order to have full access to your medical documentation, please authenticate your account on mojePZU portal. You can do it online on our portal through the mObywatel or mojeID services or go physically to one of our own PZU Zdrowie branches. Due to this, you will be able to check, in the comfort of your home:
 
referrals to see a physician,
referrals to diagnostic labs (for ultrasound, X-ray, ECG tests),
referrals to laboratory tests,
issued e-prescriptions with their PINs,
ordered tests,
recommendations from your doctor after seeing you, 
results of laboratory tests (if performed in selected own (PREMIUM) PZU Zdrowie branches or directly in collection points of Synevo or Diagnostyka labs).
 
You will also be able to add medical documentation to an appointment made on mojePZU.
 
 

Who may use the access to complete medical documentation of mojePZU?

Access to medical information and electronic referrals to see a doctor is granted to the patients who made an appointment:
via the telephone hotline or through mojePZU

How to authenticate an account on mojePZU

To authenticate an account, confirm your identity on mojePZU. You can do it: 
 
1. Online on mojePZU
 
  • via the mObywatel service
    (an option for those who have a trusted profile – how to create it, read here
1. Go to moje.pzu.pl and log into the service.
2. Go to the Medical documents tab and on the screen, click “Go to authentication”.
3. If you have the mobile mojePZU app and a trusted profile connected to it, select the mObywatel authentication method.
4. Click “Generate a QR code”
5. If you use a desktop computer, you will see a generated QR code on the screen. Open mIdentity, and then in the bottom left corner, click “Send”.
6. On the “Send data” screen, select “to an Institution or company”. A screen will open with a view from a camera with which you will scan the code from the computer screen. 
7. After scanning the code, click “Ready”. Your identity has been confirmed and you will be automatically redirected to details of your selected appointment. 
8. If you use a mobile device with iOS or Android system, you will see a generated code on the device’s screen. Click “Copy the code”, then open the mObywatel app and click “mIdentity”. In the bottom left corner, click “Send”.
9. On the “Send data” screen, select “to an Institution or company”.  A screen will open with a view allowing for pasting the copied code. 
10. After entering the code, click “Ready”. Your identity has been confirmed and you will be automatically redirected to details of your selected appointment.
 
  • via the mojeID service
    (option for those who have an account at one of the following banks: Bank Pekao, ING Bank Śląski, Bank Spółdzielczy from the PBS Group, mBank, Getin Bank, BOŚ Bank, BNP Paribas Bank Polska, Bank Spółdzielczy w Brodnicy, Credit Agricole, Santander Bank Polska, Alior Bank).
 
1. Go to moje.pzu.pl and log into the service.
2. Go to the Medical documents tab and on the screen, click “Go to authentication”.
3. Select the mojeID authentication method
4. After being redirected to the website of the National Clearing House – select one of the displayed banks where you have an account with an active electronic banking service.
5. Log into your electronic banking.
6. On the bank’s website, confirm sending data to the mojePZU portal. To confirm your identity, the bank will send your first and last name and PESEL number. The data must exactly match those given in the first step. The consent concerns provision of data to PZU Group companies which are the owners of the mojePZU portal. After positive confirmation of your identity, you will be automatically redirected to mojePZU.
 
2. At a PZU Zdrowie branch
 
During a visit at a PZU Zdrowie branch, at the reception desk, fill out a form you will receive from our employee.
