Verify your mojePZU account

Confirm your identity to gain full access to your medical records.

Browse referrals for appointments and tests, medical recommendations, and receive e-prescriptions without leaving your home.

We care about the security of our patients’ data, which is why to gain full access to your medical records in the mojePZU portal, you need to verify your account.

You can do this online in our portal using mojeID or in person at one of our PZU Zdrowie medical facilities. This will allow you to conveniently check, without leaving home:

  • referrals for medical appointments,
  • referrals to diagnostic units (for ultrasound, X-ray, ECG tests),
  • referrals for laboratory tests,
  • issued e-prescriptions along with their PIN codes,
  • ordered tests,
  • post-appointment medical recommendations,
  • laboratory test results (if performed in selected PZU Zdrowie owned facilities (PREMIUM) or directly at Synevo or Diagnostyka laboratory collection points).

You will also be able to add medical documentation to an appointment scheduled via mojePZU.

How to verify your mojePZU account

To verify your account, confirm your identity on mojePZU. You can do this in the following ways:

  • Using the mObywatel service 
    (option for those who have a trusted profile – you can learn how to create one here
1. Go to the moje.pzu.pl website and log in to the service.
2. Go to the Medical documents tab and click “Proceed to verification”.
3. If you use the mojePZU mobile app and have a trusted profile linked to it, choose the mObywatel verification method.
4. Click “Generate QR code”
5. If you are using a desktop computer, a generated QR code will appear on the screen. Open mTożsamość, then click “Transfer” in the bottom left corner.
6. On the “Transfer data” screen, select “To an institution or company”. A camera view will open, allowing you to scan the code from your computer screen. 
7. After scanning the code, click “Done”. Your identity has been confirmed and you will automatically return to the details of the selected appointment. 
8. If you are using an iOS or Android mobile device, a generated code will appear on your screen. Click “Copy code”, then open the mObywatel app and select “mTożsamość”. Next, click “Transfer” in the bottom left corner.
9. On the “Transfer data” screen, select “To an institution or company”. A screen will open where you can paste the copied code. 
10. After entering the code, click “Done”. Your identity has been confirmed and you will automatically return to the details of the selected appointment.
  • Online on mojePZU using the mojeID service
    (option for those who have an account with one of the following banks: Bank Pekao, ING Bank Śląski, Bank Spółdzielczy from the PBS Group, mBank, BOŚ Bank, BNP Paribas Bank Polska, Bank Spółdzielczy in Brodnica, Credit Agricole, Santander Bank Polska, Alior Bank, Millennium, VeloBank)
1. Go to moje.pzu.pl and log in to the service.
2. Go to the Medical documents tab and click “Proceed to verification”.
3. Select the mojeID verification method. 
4. After being redirected to the National Clearing House website – select one of the displayed banks where you have an account with active online banking.
5. Log in to your online banking.
6. On the bank’s website confirm the transfer of data to the mojePZU portal. To verify your identity, the bank will transfer your first name, last name, and PESEL. These data must match those provided in the first step. The consent concerns transferring your data to PZU Group companies, which own the mojePZU portal. After successful verification, you will automatically return to mojePZU.
  • At a PZU Zdrowie facility

During your visit to a PZU Zdrowie facility, complete the form provided by our staff at the reception.
