You have an e-referral for a laboratory test and would like to make an appointment for it.
- Go to moje.pzu.pl If you do not have an account in the portal yet, please register. If you have an account, enter your email address and password. Click Log into.
- In the menu on top of the page, select the Health tab.
- Below the main menu, on the left, select the option Make an appointment for a laboratory test.
- Fill in the form with data from the referral received from your doctor. Indicate the place and date of the test which is suitable to you. After filling out all the required fields, click Next.
- In selected cities or towns you may make an appointment for a test directly in our partner’s lab collection point. Select the lab collection point, add a scan of the referral and make an appointment. You can have the test on the date suitable for you in the period of 6 months from the date of appointment. For the test, you do not need to take the referral but remember to use the client’s code and order number we have sent you by a text message or email.
- If there is no lab collection point in your city or town or if no location is suitable for you, use the option Make an appointment for a visit through a consultant.
- Our consultant will check available dates at the time preferred by you and will contact you.
- On the appointed date, go to the test. If you have made an appointment for a test through a consultant, do not forget to take the referral.
- After the test performed in our partner’s lab collection point or at a Premium center, you may click its details and get to know the results of your tests.
If there is such a need, when you are making an appointment at a lab collection point for free-of-charge tests, you may also purchase additional tests not covered by your package or subscription. You can pay for paid services through an e-payment supplier.
If a consultant has made an appointment for a paid service for you, you will pay for it at the center.
