Do you have an e-referral for a laboratory test and want to book it?
- Go to https://moje.pzu.plmoje.pzu.pl If you do not yet have an account on the portal, register. If you already have an account, enter your email and password. Click Log in
- In the menu at the top of the page, select the Medical care tab.
- Below the main menu, select Schedule a test, and then Laboratory test.
- Fill in the form with the data from your doctor’s referral. Indicate the place and time of the test that suits you. After completing all required fields, click Next.
- In selected cities, you can book the test directly at a sample collection point of our partner. Select a collection point, upload a scan of the referral, and schedule your appointment. You can complete the test at a convenient time within 3 months of the booking date. You do not need to bring the referral to the test, but remember to use the client code and order number sent to you via SMS or email.
- At PZU Zdrowie facilities, you can schedule the test for a specific date and time.
- If there are no collection points in your city or none of the locations suit you, use the Request appointment via consultant option.
- Our consultant will check available dates at your preferred time and contact you.
- Attend the test at the agreed time. If your appointment was arranged via a consultant, remember to bring your referral.
- After the test performed at a partner’s collection point or at a Premium facility, you can click on its details to view your test results.
If needed, when booking at a collection point for free tests, you can also purchase additional tests not covered by your package or subscription. Paid services can be settled via an online payment provider.
If a consultant scheduled a paid service for you, you will pay for it at the facility.
