You have received from a doctor a referral for ECG, ultrasound, X-ray, CT, MRI, outpatient procedure or rehabilitation and you would like to make an appointment for it.
- Go to moje.pzu.pl If you do not have an account in the portal yet, please register. If you have an account, enter your email address and password. Click Log into.
- In the menu on top of the page, select the Health tab.
- Below the main menu, on the right, select the option Make an appointment for a test.
- Fill in the form with data from the referral received from your doctor. Indicate the place and date of the test which are suitable to you. After filling out all the required fields, click Next.
- If a selected center presents a schedule of visits, click Available Date. Then, select the preferred time of the visit and click Make an appointment for a visit. On the screen, you will see a summary of the appointed test.
- Otherwise, select the center in which you would like to have the test performed and on the right click Book a visit.
- Specify in detail the place and date of the test and the way in which the consultant is to contact you about available dates. Click Make an appointment.
- On the appointed date, go to the medical center. Do not forget to take the referral.
