You may file a complaint in a way chosen by you.

Fill out the online form.

Send an e-mail to reklamacje@pzu.pl.

Send a letter to the address: PZU, ul. Postępu 18, 02-676 Warsaw.

Visit our Medical Center and file a complaint directly in the center.

Call – 801 102 102 (charges for calls in accordance with the operator’s rates).

In your complaint, include data which will make it possible to identify the customer and the reported problem:

  • first and last name/business name of the author of the complaint,
  • PESEL/REGON number,
  • address of the company’s registered office,
  • policy/claim/RIN/CBUZ no.,
  • telephone no./email address,
  • description of the subject matter of the complaint.
