You may make an appointment for occupational medicine tests on PZU Zdrowie’s information hotline Monday to Friday, from 8:00 am to 6:00 pm. It is enough if you call us – 801 405 405 (charges for calls in accordance with the operator’s rates). Our consultants will help you make an appointment convenient for you.
Before the conversation, prepare a referral to tests. During the conversation, we will ask you for data from a referral:
- first and last name,
- PESEL,
- type of tests (preliminary tests, follow-up, periodic, reimbursement of costs of glasses),
- your employer’s name and REGON (statistical number),
- date of issuing the referral,
- the final date by which the tests should be performed,
- workstation,
- harmful factors.
Remember to hold your identity document and referral on the date of the visit. After the tests, you will receive from the doctor the original and a copy of a certificate. Give the original to your employer, and keep the copy for yourself.
